i kept my shoulders over he bar or just behind he bar and at first worked on facing straight ahead instead of looking up, which all went to shit on the last set of 405. i could use some advise on my form.

I'm probably the last person that should be giving anyone advice on form but anyway...
In your other post you mentioned "it seems like my hips come up too soon"
I think they do too. Only reason I notice is because that is something I really have to pay attention to.
Maybe think about pushing the floor away with heels and kinda lead with the head and shoulders. I don't know.
Here is mine from June. Obviously yours looks alot better than mine but maybe some of the advice I got could be helpful maybe.

Your form is always subject
Your form is always subject to break down when lifting such heavy weight. Unless it's keeping you from improving on your numbers, I wouldn't worry about it. Your form looks fine anyways.

I don't think it looked that bad. The set up took about 20 sec. until the bar started to move. If you want to see some really "bad form" watch Konstanovics DL 939 lb. RAW.
